Medications:

An adequate supply of personal prescription medications
Antihistamine (for allergies)
Benadryl, Claritin, Chlortrimaton or generic equivalents
Decongestant, alone or in combination with an antihistamine

Sudafed, Afrin or generic equivalents
Motion sickness medication
Dramamine, Marezine, Transderm Scop (Rx only) or generic equivalents
Pain or fever relievers
Tylenol, Advil, Aleve or generic equivalents
Antidiarrheal medication
Immodium, Pepto Bismol or generic equivalent
Mild laxative
Metamucil products, Correctol, Sennekot or generic equivalents
Cough suppressants
Robitussin DM, Delsym, or generic equivalents
Antacids

Antifungal topical products
Neosporin, Polysporin or generic equivalents
1% Hydrocortisone cream (topical steroid for itching & allergic reaction)

Depending on travel destination, it may also be important to pack antimalarial medications, high-altitude sickness medication &/or antibiotics for self-treatment of moderate to severe traveler’s diarrhea.
Other important items
Saline nasal spray
Lip moisturizer with sunscreen
Moisturizing eye drops
ie: Natural Tears
Thermometer
Sunscreen (SPF of at least 15)
Insert repellant with DEET (used on exposed skin)
Insert repellant with permethrin (used on clothing)
Duplicate sets of keys
ie: home, car, luggage - kept in separate places
Important documents
Insurance or Medical cards
Copies of prescriptions, including eye glasses
Note: Certain prescription medications should be carried together with a medical attestation, signed by a physician, certifying that the traveler requires the medication. Some countries require additional signatures or documentation.
All medications should be carried in the hand luggage to minimize any risk of loss during the journey. A duplicate supply carried in the checked luggage is a safety precaution in case of loss or theft.
Identification:
Passport
Driver's License
or Photo ID card
Name, address, & any pertinent health information on a card in the language of area you are traveling
Traveler's Checks & list of numbers
List of telephone numbers for credit cards & banks
Official documentation of vaccination status & a dupilcate stored separately
